SSD: I mailed in my Continuing Disability Review and received another packet, what's this mean?

I was selected by Social Security for a "CDR" after 6 years on disability for schizo-affective disorder, which I then filled out and mailed in promptly. A month later, I received another packet in the mail, it asks more about my illness, and it also wants to know my previous work history for the 15 years before I received disability. This packet is addressed to return to London, KY, where as the previous packet was addressed to my local department in NY. The funny thing is, the packet I received doesn't ask for doctors information or have a due date on when it needs to be returned, it doesn't say specifically what it is for like the CDR says it's a review, this one doesn't say anything thing other than "complete this questionnaire" and it has the packet to return it in. Is this legit? Is this a sign of something bad, neutral, or good? Does this mean I will be getting a face to face interview or being denied? Also, the packet has a section specifically asking about my anxiety. My anxiety does inhibit my ability to work, but the main problem is the schizo-affective disorder, how can i make that obvious with out downplaying the anxiety or seeming too capable? Thanks!
